What is exuberant granulation tissue?
Exuberant granulation tissue is essentially an overgrowth of granulation tissue, which is a normal phase in wound healing. The proliferation of excess granulation tissue prevents the wound from healing properly. There are various causes and predisposing factors.
How is exuberant granulation tissue treated?
In most cases treatment of exuberant granulation tissue is simple and surgical excision appears to be the best choice. Surgical excision should occur as soon as the granulation tissue protrudes above the wound margins. Excision can be performed while the horse is standing.
What is Proudflesh?
When granulation tissue grows out and protrudes from the wound, then the granulation tissue is known as proud flesh. Proud flesh is not conducive to wound healing as it prevents the wound from epithelializing (the process of the skin cells covering the wound).
What causes over granulation?
It is possible that overgranulation occurs due to the over-efficiency of modern day dressings or a possible infection that has prevented the ‘switching-on’ day following surgery (Collins et al, 2002). Due to the surgery and his poor general condition, the patient had lost more than two stone in weight.
When should granulation tissue be removed?
It is recognized by a friable red to dark red, often shiny and soft appearance, which is raised to the level of the surrounding skin or higher. This tissue must be removed in order for re-epithelialization to occur.
What does Hypergranulation look like?
Hypergranulation is characterised by the appearance of light red or dark pink flesh that can be smooth, bumpy or granular and forms beyond the surface of the stoma opening. 137 It is often moist, soft to touch and may bleed easily. It is normal to expect a small amount of granulation around the site.
How can we reduce granulation?
TREATING OVERGRANULATION In an overgranulated wound, the use of a dressing that promotes granulation should be stopped and changed to one that provides a warm moist environment, reduces overgranulation and promotes epithelialisation, such as a foam dressing.
Does granulation tissue bleed?
This gives granulation the appearance of red lumps (or granules) within the normal paler pink matrix, and despite this pink to red appearance, granulation does not bleed easily. Once the wound is filled with granulation tissue, it provides a foundation for re-epithelialization.
Exuberant granulation tissue typically is an irregular and unhealthy appearing tissue with many grooves and clefts. Histologically exuberant granulation tissue has remains of fibrin deposits, which have not been cleared by the acute inflammatory response, chronic inflammation has developed and proliferation is very active.
